About the Opportunity

An opportunity exists for a dedicated Registered Nurse – Palliative Care to join the Generalist Care multidisciplinary team based at Taree Community Health Service. In this rewarding community-based role, you will deliver specialist palliative nursing care through home visits, supporting patients and families across the Lower Mid North Coast.

 

About the Role

As a Community Palliative Care Registered Nurse, you will:

  • Deliver compassionate, person-centred care to individuals with life-limiting illnesses in their homes
  • Provide specialist symptom management and medication management
  • Coordinate care and manage a clinical caseload with confidence and autonomy
  • Work collaboratively with the Lower Mid North Coast Specialist Palliative Care team to address physical, emotional and social needs
  • Support patients and families to maintain dignity, comfort and quality of life

You will work both independently and as part of a highly supportive team, requiring strong clinical judgement, prioritisation skills and clear communication with patients, carers and multidisciplinary partners.

 

About the Team

You will be part of the Generalist Care multidisciplinary team, which includes:

  • Wound Management & Wound Specialist
  • Cardiac & Respiratory Rehabilitation
  • Continence
  • Dietetics
  • Diabetes Education
  • Immunisation
  • Audiometry

Within your immediate work environment, you will collaborate closely with:

  • Community Health Nursing
  • Nurse-Led Clinic
  • Child and Family Services
  • A range of outreach services

The dedicated Palliative Care team consists of a Clinical Nurse Consultant, Nursing Staff, Bereavement Counsellor and Palliative Care Volunteers. Teams are based to support four areas across the Lower Mid North Coast: Taree, Forster, Gloucester and Bulahdelah.

 

About You

You are a compassionate and resilient AHPRA approved Registered Nurse with:

  • A genuine passion for palliative care and holistic, person-centred practice
  • Strong communication and interpersonal skills
  • Excellent organisational ability and confident caseload coordination
  • Critical thinking skills and commitment to evidence-based practice
  • The ability to work effectively within a multidisciplinary team
  • Confidence navigating the complexities of community-based nursing

You hold postgraduate qualifications in Palliative Care or are willing to work towards them.

 

Support & Professional Development

You will be supported by:

  • Nurse Unit Manager
  • Palliative Care Clinical Nurse Consultant
  • Wound Management Specialist (for specialty tasks)
  • Clinical Nurse Educator

You will attend annual clinical competencies in Syringe Drivers and other relevant skill areas. Ongoing education and professional development opportunities are encouraged and supported in alignment with the core business of the Community Palliative Care Nursing team.
