About the role
Join a collaborative, client-centred team making a meaningful difference in the lives of older people across the Lower Mid North Coast. As an Aged Care Assessor within the Single Assessment Service, you will conduct ACAT assessments in community and hospital settings. You’ll evaluate individual care needs and determine eligibility for government-subsidised aged care services, supporting older people to access the right care at the right time. Assessments are completed using a mobile computing environment, enabling flexible, on-the-go clinical work.
What you’ll be doing
- Conduct comprehensive ACAT assessments across community and hospital environments
- Evaluate client needs and recommend appropriate aged care services
- Work within a mobile digital platform to document and manage assessments
- Collaborate closely with multidisciplinary services to ensure holistic care planning
Your team
You’ll work alongside a dedicated group of ACAT Assessors and partner with a wide multidisciplinary network including:
- Transition Aged Care Program (TACP)
- Dementia Support and Psychogeriatric Services
- Podiatry, Occupational Therapy, Speech Pathology & Physiotherapy
Positions are well supported by an experienced Administration Team and Health Service Manager.

If you’re not yet qualified as an ACAT Assessor, we will support you with the required training. Successful applicants will complete the Single Assessment Training for ACAT Assessors and receive ongoing professional support. A passion for continuous professional development is essential.

Requirements:
Candidates must hold one of the below:
- Current registration as a Registered Nurse with AHPRA.
- Must hold as a minimum a bachelor’s degree in social work which provides eligibility for membership of the Australian Association of Social Workers, or other qualification deemed equivalent by the employer.
- Must hold qualifications recognised for registration with the Physiotherapy Board of Australia. Must hold general registration with the Physiotherapy Board of Australia.
- Must hold qualifications recognised for registration with the Occupational Therapy Board of Australia. From 1 July 2012, must hold general registration with the Occupational Therapy Board of Australia.
- Must hold a four-year degree in psychology, being a three-year degree with a fourth-year honours in psychology, or a qualification deemed equivalent by the employer, and be registered (general) or eligible for registration as a Psychologist with the Psychology Board of Australia via AHPRA.
