Join the Hospital in the Home (HITH) team at Taree Community Health and deliver high-quality, patient-centred care in the comfort of patients’ homes. You’ll play a vital role in preventing hospital admissions and supporting early discharge, helping patients recover safely while staying connected to their community.
About the Team
You’ll be part of a dedicated and supportive HITH team within a broader multidisciplinary service, working alongside Community Nursing, Palliative Care, Cardiac and Respiratory Rehabilitation, Continence Services, Dietetics, Diabetes Education, Wound Management and Audiometry. Collaboration and holistic care are at the heart of what we do.
About the Role
This is a challenging and rewarding opportunity for a Registered Nurse who enjoys autonomy, variety and meaningful patient interactions. You’ll work within a small, close-knit team on a seven-day roster, with a mix of 12-hour and 8-hour day shifts.

We’re seeking an experienced and highly skilled Registered Nurse with:
Excellent communication and interpersonal skills
Strong critical thinking and evidence-based practice
The ability to deliver individualised, person-centred care
Excellent organisational and time management skills
A caring, motivated attitude and positive team ethic
How You’ll Be Supported
You’ll be supported by an experienced Nurse Unit Manager and a network of skilled professionals. Specialist support is available from the Wound Management Specialist and Clinical Nurse Educator, with ongoing education and training opportunities aligned to the core business of the HITH service.
