Central Coast Local Health District (CCLHD) is looking for a skilled and committed Permanent Part Time Clinical Nurse Specialist Grade 2 (CNS 2) to oversee care coordination and contribute to the delivery of integrated, patient-focused healthcare.

 

About the Role

The CNS 2 supports an integrated approach to care for patients with chronic and complex care needs through care coordination.
 
The care coordination role can involve:

  • being guided by the individual care needs of the patient
  • liaising and linking with multiple services
  • supporting both patient and carer/family to self-management
  • addressing barriers to engagement, ensuring a sustainable ongoing care plan is established
  • Working Hours: Part time between Monday and Friday, 8:00 AM – 4:30 PM

About You

  •        Current registration with AHPRA as a Registered Nurse (Division 1) with relevant post-registration qualifications and at least 3 years’ experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation.
  •        Evidence of clinical leadership skills and the ability to work collaboratively within a multidisciplinary framework to plan, deliver and coordinate patient care.
  •        Demonstrated ability to apply advanced clinical expertise in relation to clinical nursing practice and independently make decisions that support good patient care outcomes.
  •        Valid NSW Drivers' licence and the willingness to travel in the course of employment. (please upload a copy to your application)
