We are seeking a dedicated and experienced Clinical Nurse Specialist Grade 2 to join our Supported Transitioning Care Team (STCT). In this pivotal role, you will provide clinical leadership to Registered Nurses within the STCT and ensure the delivery of high-quality, person-centred nursing care that supports safe and effective discharge planning for patients and families facing complex transfer-of-care risks.
This position involves delivering complex specialist nursing care and driving the development of clinical practice standards across the team. You will play a key role in enhancing patient outcomes through evidence-based care and support, ensuring continuity and safety during the transition from hospital to community or alternative care settings.
Key Responsibilities

Provide expert clinical leadership and guidance to STCT nursing staff.

Coordinate and implement safe discharge plans for patients with complex needs.

Support families and carers through the transition of care process.

Drive continuous improvement in service delivery and patient care standards.

Contribute to education, mentorship, and professional development of nursing staff.

Collaborate with multidisciplinary teams and community services to optimise patient outcomes.

About You
- Current registration with AHPRA as a Registered Nurse (Division 1) plus post graduate qualifications relevant to the specialty and at least 3 years fulltime equivalent clinical experience in the specialty of the specified post graduate qualification.
- Demonstrated ability to provide complex care and advanced clinical skills.
- Demonstrated ability to work effectively and collaboratively both independently and as part of the multidisciplinary health care team.
