We are seeking a dedicated and experienced Clinical Nurse Specialist Grade 2 to join our Supported Transitioning Care Team (STCT). In this pivotal role, you will provide clinical leadership to Registered Nurses within the STCT and ensure the delivery of high-quality, person-centred nursing care that supports safe and effective discharge planning for patients and families facing complex transfer-of-care risks.

This position involves delivering complex specialist nursing care and driving the development of clinical practice standards across the team. You will play a key role in enhancing patient outcomes through evidence-based care and support, ensuring continuity and safety during the transition from hospital to community or alternative care settings.


Key Responsibilities

  • Provide expert clinical leadership and guidance to STCT nursing staff.

  • Coordinate and implement safe discharge plans for patients with complex needs.

  • Support families and carers through the transition of care process.

  • Drive continuous improvement in service delivery and patient care standards.

  • Contribute to education, mentorship, and professional development of nursing staff.

  • Collaborate with multidisciplinary teams and community services to optimise patient outcomes.

About You

  • Current registration with AHPRA as a Registered Nurse (Division 1) plus post graduate qualifications relevant to the specialty and at least 3 years fulltime equivalent clinical experience in the specialty of the specified post graduate qualification.
  • Demonstrated ability to provide complex care and advanced clinical skills.
  • Demonstrated ability to work effectively and collaboratively both independently and as part of the multidisciplinary health care team.
