NBMLHD Community Mental Health offer a range of specialised services for children, young people, families and older people, through our community-based services and health centres. Our multidisciplinary health professionals and case managers take a collaborative approach to mental health care by linking with their patients to work together and consult within an environment appropriate to the person's needs.
Our Penrith Community Mental Health team is seeking to engage a Senior Clinician in the role of Integrated Care Coordinator. The position aims to provide clinical leadership within the Penrith Community Mental Health Team with advanced scope of practice to support integrated care coordination. The successful incumbent will ensure provision of expert clinical skills and knowledge to members of the multidisciplinary team providing care coordination support to consumers of the service.

What you will bring to the role:
- Registered Nurse (Division 1) with AHPRA with post-registration qualifications in Mental Health and at least 3 years' experience working in Mental Health, or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation.
- Demonstrated skills in mental health and discipline specific assessments and interventions and experience working with people with mental illness.
- Demonstrated ability to provide leadership to a multidisciplinary team across services and settings including skills and experience in clinical support planning and integrated care coordination.
- Demonstrated organisational skills and ability to manage a complex caseload with competing priorities
- Demonstrated ability to collaborate meaningfully with consumers and carers with a commitment and understanding of recovery principles in mental health service provision
- Demonstrated high level written and verbal communication skills and effective time management skills
- Current Driver's Licence and willingness to use for work purpose travel
