The Role

As a Registered Nurse - Community Dementia Support, you will provide safe, compassionate, and person-centred care for clients living with dementia in the community. Collaborating with clients, families, carers, and the multidisciplinary team, you will help achieve individual goals and outcomes. This role focuses on supporting not only the person with dementia but also their caregivers, offering education, counselling, and advocacy to assist them in making informed decisions. You will work closely with the Memory and Cognitive Clinical Nurse Specialist, support Geriatrician clinics, conduct comprehensive cognitive screenings, and respond effectively to emergency situations. Responsibilities also include episodic care coordination, performing ACAT assessments, and maintaining accurate documentation within electronic systems.

The successful applicant will mentor new staff, supervise nursing students, and promote the use of digital platforms, supporting clients and carers in adapting to these technologies. You will receive robust support through orientation, clinical supervision, Northern Clinical Networks meetings, and ongoing mentoring. Requirements
  • Current registration as a Registered Nurse with AHPRA.
  • Eligibility to drive in NSW and the ability and willingness to travel for work purposes which may involve long distances.
