  • You are a motivated Enrolled Nurse who can take initiative in community settings.  
  • You are skilled in interpersonal and communication, allowing for effective consultation with a multi-disciplinary team.  
  • You maintain accurate detailed patient reports and records to meet legislative requirements and ensure continuity of safe patient care delivery
  • You are able to support and inform patients/clients and their families to ensure they understand clinical interventions and service delivery, helping them make informed decisions about their health needs.
  • You are capable of collaborating effectively as part of a dynamic team.  


About the role:

Join our dedicated team in the Transitional Aged Care Program (TACP), a crucial Commonwealth-funded initiative that supports older adults as they transition from hospital to home. This short-term program, lasting up to 12 weeks, is designed for individuals who may be eligible for ongoing care in a residential aged care facility but have the potential to regain their independence.

In this fulfilling role, you will provide personalised care and support to enhance clients' physical, cognitive, and emotional well-being, helping them thrive in the comfort of their own homes. You’ll play a key role in fostering recovery and promoting autonomy, making a significant impact on the lives of older Australians at a pivotal point in their journey. This casual position will see you working as part of the Transitional Aged Care team, alongside Enrolled Nurses, Registered Nurses, Physiotherapists, Occupational Therapists and Domestic Assistants. You will be responsible for the assessment and care of clients to assist them to achieve their goals. This program runs as a 7 day/week service so will include some weekend work. Face-to-face training will be provided to transition into the role and support will be available from RN’s, EN’s, NUM and Service Manager in the position.

Requirements:

Our successful candidate will possess the following:

  • Current registration as an Enrolled Nurse with AHPRA
  • Unrestricted Driver's License and willingness to travel within the community
