About the role:
An exciting opportunity exists for an Aboriginal Health Practitioner/Worker to join our Integrated Chronic Care for Aboriginal People Program. This position is responsible for the provision of culturally safe, patient centred services to Aboriginal clients and their families with chronic or complex needs by working with a Clinical Nurse Consultant to provide Assessment, care coordination and education to clients, families and their carers that considers clinical, psychosocial, and cultural needs.
Where you will be working:
This temporary full-time position is based in Inverell and provides outreach services to Ashford, Tingha, Glen Innes, Tenterfield, Armidale, Uralla, and Guyra and works as part of a collaborative team in supporting and coordinating care with ongoing engagement of Aboriginal clients and carers who have chronic and complex needs including diabetes, cardiac, respiratory, kidney and cancer. This position will require a motivated, enthusiastic, and conscientious Aboriginal Health Practitioner / Worker, who has the ability to work in partnership with the Clinical Nurse Consultant and as part of the broader team.
The successful applicant will be supported by the Integrated Chronic Care Clinical Nurse Consultant, their Line Manager and the broader ICCAPP and Aboriginal Chronic Care Program Teams.
