This position qualifies for a Recruitment Incentive Package up to the value of $10,000 (pro-rata part-time/temporary positions less than 18 months), and a reoccurring Annual Retention Bonus valued up to $10,000 (pro-rata part time), under the NSW Health Rural Health Workforce Incentive Scheme. This scheme recognises the role key health workers play in enabling the delivery of rural and remote health services, and aims to provide incentives over and above award entitlements, to attract and retain staff in these areas.

To recognise ongoing service in this role, once the successful candidate has served a qualifying period, they will become entitled to a reoccurring Annual Retention Bonus valued up to $10,000 (as taxable income).

What you'll be doing
  • Provide assertive support and post discharge follow up to consumers who are transitioning between acute mental health care and the community.
  • Work to actively reduce the 28-day readmission rate for consumers leaving acute mental health care by providing peer support for up to 6 weeks following discharge.
  • Offer hope to consumers who are leaving acute mental health care, facilitates contact with people who have a personal lived experience of recovery and firsthand experience in wellbeing management.
  • Provide support to consumers for Community Treatment Orders and Mental Health Review Tribunal hearings. Peer-STOC peer workers will provide assertive in-reach for consumers in acute mental health units in the 7 days prior to their discharge. 
  • Establish relationships, build rapport, discuss wellbeing strategies and collaboratively identify supports needed to transition into the community.
  • Provide direct face-to-face follow up, preferably with the case manager, on the day directly following discharge, and no later than 72 hours after discharge.
  • If consumer can’t be located, the Peer-STOC worker will escalate to the case manager or Team Leader CMHDA.
  • Support will be provided for up to 6 weeks following an acute admission. 
  • Priority for referral should be given to consumers who have no formal identified community supports at the point of discharge. Existing support includes care coordination through Community MHDA team, HASO, HASI Plus, PHAMS, NDIS etc.

  1. Certificate IV in Mental Health Peer Work or willingness and commitment to obtain Cert IV in Mental Health Peer Work.
  2. Personal lived experience of mental distress and ability to meaningfully and purposefully use this lived experience to provide peer support.
  3. Applied understanding of the principles of person-centred care, trauma-informed care and recovery, as well as in working with Aboriginal people in a holistic, collaborative and respectful way.
  4. Experience in working collaboratively with individuals and groups including the multidisciplinary team; and ability to build and maintain effective relationships with external providers, consumers and carers.
  5. Demonstrated effective written and verbal communication skills.
  6. Demonstrated experience in utilising multiple computer applications including Microsoft Office suite and government systems.
  7. Evidence of a Class C Drivers Licence and ability and willingness to travel for work purposes.
