Role title: Pathways to Community Living Initiatives (PCLI) Coordinator
Role type: Max term contract (until March 2027), full time, Monday-Friday (8:30am-5pm)
Role grade: Occupational Therapist, Level 4 (Year 1-2)

Salary: $126,524.00 - $129,591.00 + Super + Salary Packaging

About the role
- The Pathways to Community Living Initiative (PCLI) is a key program of the NSW Mental Health Reform, established to address the needs of people who have had long mental health inpatients stays (over 365 days) or those who are at risk of long stays, to enable consumers to transition to meaningful lives in the community
- Provide expert clinical advice to clinicians across all teams of Mental Health, and consumers to provide expert and continuity of service for patients who meet the criteria for PCLI
- Develop, facilitate, implement and evaluate collaborative multiagency care plans for long stay mental health consumers with complex needs within the mental health facility
- Operate according to the principles of harm minimisation, trauma-informed care and the strengths model
- Support teams in utilising assessment outcomes in routine service planning and review, triaging of new referrals and providing consultation liaison services with internal and external agencies
- Participate in multidisciplinary Clinical Review Meetings and coordinate case conferences to ensure delivery of safe, effective and collaborative health care
About you
- Current unrestricted drivers license
- Relevant tertiary qualifications and current registration with the Australian Health Practitioner Regulation Agency (AHPRA) for Occupational Therapists
- Demonstrated advanced clinical skills in mental health and discipline specific assessments and interventions, experience working with people with severe and persistent mental illness
- Knowledge and understanding of the Mental Health Strategic Plans, and Policy Frameworks which impact on current mental health practice (including PCLI)
- Demonstrated capacity to function at an advanced clinical level and to provide clinical leadership and supervision, within a multi-disciplinary treatment setting and share professional expertise with colleagues and service partners to enhance recovery-oriented service delivery
- Demonstrated ability to champion practice improvement through participation in quality improvement, service development, education and training, and other activities to facilitate continual improvements in the quality and safety of care
