We are seeking a committed Pharmacist to join our Hospital in the Home team. The position ensures the provision of clinical pharmacy services to patients referred for support. In this role, you will provide comprehensive medication reviews, counselling, and drug information to patients, ensuring a safe and seamless transition from hospital to home. You will play a vital part in coordinating care between the hospital team and general practitioners, supporting continuity of treatment and patient wellbeing.
The Pharmacist carries the responsibility for managing medication ordering and timely supply, as well as providing essential drug information to Hospital in the Home (HiTH) nurses to enable the safe administration of intravenous medications in patients’ homes. In addition, you will offer key advice to medical teams on admitted patients, supporting decisions to step down from intravenous to oral medications and facilitating prompt, safe discharges.

What you will bring to the role:
- Full pharmacist registration with Australian Health Practitioner Regulation Authority (AHPRA) with no restriction to scope of practice.
- Demonstrated pharmacy experience in the provision of hospital and clinical pharmacy service including care coordination between the hospital team and the GP.
- Demonstrated ability to work independently and cooperatively in a multidisciplinary team environment.
- Demonstrated excellent communication skills written and verbal with the ability to communicate issues relating to medication safety.
- Demonstrated understanding of quality improvement processes.
- Demonstrated experience in clinical pharmacy preferably within tertiary Australian Hospital.
- Current driver's licence and willingness to use for work purpose travel
