About the Opportunity
The Project Officer, Spinal Cord Injury Model of Care - Discharge and Community Reintegration will lead and support the design, implementation, monitoring and evaluation of strategic initiatives that advance the statewide delivery of the Spinal Cord Injury (SCI) Model of Care across NSW.
Working collaboratively with the project team, the State Spinal Cord Injury Service (SSCIS) Network Manager, clinicians, and key stakeholders, the role will strengthen integration between specialist SCI services, non‑specialist hospitals, and community‑based supports to improve patient outcomes and continuity of care.
The position will play a critical role in establishing and testing innovative care pathways, including intensive early discharge planning and community integration models, ensuring evidence‑based practice, consistent implementation, and measurable impact. Acting as a bridge between hospital and community settings, the role will support capability development in non‑specialist services, facilitate peer‑supported patient education, and contribute to system‑wide improvements that enable people with spinal cord injury to live safely and independently in the community.

Selection Criteria
- Tertiary qualifications in allied health or nursing with at least 5 years' experience in the management of people with spinal cord injury or related conditions in the community.
- Experience in working with a range of stakeholders in the disability sector.
- Demonstrated ability to provide a consultancy service for complex cases.
- Proven ability to implement change and introduce innovative ways to create safe and sustainable discharge pathways.
- Experience in project management.
- Demonstrated ability to document, analyse and report on complex information rapidly.
- Ability to work flexibly and pivot to achieve outcomes.
- Demonstrated ability to effectively communicate at every level of the stakeholder partnership.
