What you'll be doing
Out of Hospital Care (OHC) incorporates the ComPacks, Safe and Supported at Home (SASH) and End of Life (EoL) programs. OHC supports hospital discharges and prevents avoidable admissions by providing short and medium-term packages of non-clinical care.
The Out of Hospital Care Coordinator is the central point of coordination for MNCLHD, ensuring smooth transitions across the continuum of care by facilitating effective and sustainable initiatives, building strong collaborative relationships, and providing leadership and guidance to services supporting clients accessing Out of Hospital Care packages.

About you
Do you have qualification in one of the following: Clinical Psychologist (Masters in Clinical Psychology), or Registered Psychologist with a 4-year accredited psychology degree; Registered Physiotherapist (Physiotherapy Board of Australia); Occupational Therapist with qualifications recognised by the Occupational Therapy Board of Australia; Bachelor level Counsellor; Social Worker Degree eligible for AASW membership; Dietitian with a Nutrition and Dietetics degree eligible for full DAA membership; Speech Pathologist with eligible qualifications for Speech Pathology Australia membership; Diversional Therapist with a degree recognised by DTA; Community Welfare degree or relevant qualifcation for Welfare officer role and demonstrated clinical knowledge in caring for clients with broad scope of comorbidities, health and care needs including dementia and end of life.
