An exciting opportunity exists for an experienced and motivated Registered Nurse to step into a pivotal leadership role as Neurology Case Manager within the multidisciplinary Neurology and Stroke team at John Hunter Hospital.

This role is central to coordinating high-quality, evidence-based care for patients with complex neurological and stroke conditions, while supporting patient flow, service coordination, and clinical excellence across the ward.


About the Role

As the Neurology Case Manager, you will work closely with the Neurology and Stroke teams to ensure seamless patient journeys from admission through discharge or transfer. You will play an active role in clinical decision-making, ward rounds, and multidisciplinary collaboration, while also acting as a key point of contact for patients, families, and staff.

This position reports to the Nurse Unit Manager (NUM), Ward G2, and offers the opportunity to contribute meaningfully to service improvement and staff development within a supportive team environment.

 Key Responsibilities

  • Coordinate patient flow and manage referral pathways to other facilities, private hospitals, or return to local areas

  • Deliver evidence-based acute neurology care in line with approved Clinical Pathways and guidelines

  • Actively participate in inpatient ward rounds with Neurology and Stroke teams

  • Communicate effectively with the multidisciplinary team, including:

    • G2 NUM

    • Stroke Coordinator

    • Neurology and Stroke Outpatient Coordinators

    • Ward Nursing Staff

    • Physiotherapist, Occupational Therapist, Speech Pathologist

    • Social Worker and Dietician

  • Provide education, direction, and clinical support to junior staff and team members on the Neurology ward

  • Serve as a supportive and knowledgeable point of contact for patients and their families
     

Essential Qualifications & Requirements

  • Current registration as a Registered Nurse with AHPRA

  • Minimum Postgraduate Certificate relevant to Neurology, Stroke, or a related specialty

  • Demonstrated experience caring for Neurology and Stroke patients

  • Experience coordinating patient flow and managing complex referrals

  • Advanced case management or care coordination experience

Leadership or mentoring experience within an acute care setting
