Remuneration: Dependent on qualifications

What you'll be doing
This position is a multidisciplinary position and open to registered nurses, social workers, occupational therapists, physiotherapist and speech pathologists. A hands-on, decision-making role where your clinical judgement will directly influence the care and services older people receive.
You will:
Deliver in-depth aged care assessments across medical, functional, cognitive and psychosocial domains
Determine eligibility for Commonwealth funded aged care services under the Aged Care Act
Build clear, person-centred support plans that work in the real world
Partner with families, carers, clinicians and community providers
Maintain high-quality clinical documentation and meet key reporting timeframes
This is a temporary role, subject to extension based on Commonwealth decision tender and market share. this position is a multidisciplinary position and open to registered nurses, social workers,
occupational therapists, physiotherapist and speech pathologists.

Essential Criteria:
Qualification and Registration: Tertiary qualification & current registration with AHPRA in Nursing, Physiotherapy, Occupational Therapy or tertiary qualification in Social Work with eligibility for membership of the Australian Association of Social Work or Speech Pathology with eligibility for membership of Speech Pathology Australia.
Selection Criteria:
Demonstrated post graduate experience working with older people in an acute or community setting.
Demonstrated ability to work independently & in an effective manner encompassing appropriate initiative, organisational, problem solving & time management skills.
Proven written & verbal communication skills including demonstrated computer literacy.
Demonstrated ability to work autonomously as well as function as a member of a multidisciplinary team.
Understanding or experience in the health & functional support needs of older people & their carers in the community setting.
Current Drivers Licence (minimum class C) valid within Australia & demonstrated competent driving ability/ experience.
