War Memorial Hospital (WMH) is a 3rd Schedule Aged Rehabilitation hospital, situated in Waverley, owned and operated by Uniting and part of the South Eastern Sydney Local Health District (SESLHD). Services are comprised of 24-hour inpatient care, Monday to Friday outpatient departments, iREAP Day Rehabilitation program, several specialist community teams inclusive of Transitional Aged Care, Aged Care Assessment Team, Geriatric Flying Squad, Young Onset Dementia, and day centre program, 24-hour accommodation service for regional guests and the full range of hospital support services. The Geriatric Flying Squad provides advanced clinical assessment, investigation and develop management plans for older people in the community and in residential aged care facilities over 7 days per week. This service aims to maintain the health and independence of older people living in residential care and the community by providing rapid access to acute aged care nursing for older people experiencing a decline or acute changes in their condition whilst remaining in their RACF or in their home. This is via the provision of a multi-disciplinary geriatric assessment and short term case management. The aim of the GFS is to (where appropriate) implement hospital avoidance strategies who are at high risk of emergency department presentation or readmission to hospital.

The Aged Care Nurse Practitioner (NP) is an integral member of the Geriatric Flying Squad (GFS), co-ordinating and delivering high quality nursing care to patients/clients in our community. This will be an effective strategy for keeping older people well in their usual residence, reducing avoidable hospital presentations and/or admissions and supporting the older person to remain in a familiar environment. Working well within a multidisciplinary team of nursing staff, allied health and medical professionals is essential.

This advanced practice role includes comprehensive assessment, interpretation of diagnostics, implementation of treatment modalities and relevant referral skills applied to a broad range of challenging, complex and changing situations within the SESLHD community and RACF setting.

The NP will work autonomously and in collaboration with multidisciplinary team, under the direction of the GFS Head of Department and seeking clinical support and advice from the Geriatricians on-call where appropriate. The position utilises existing pathways and establishes new pathways to ensure provision of care will be delivered in the setting most appropriate for the client. The casual position enables continuity of care during periods of planned and unplanned leave within the GFS team.

The NP will utilise an evidence base, contemporary theories of practice and clinical coaching approach to foster a learning environment and facilitate clinical and professional development of nurses across the WMH campus. The NP/TNP will be pro-active with incorporating education provision to RACF staff with a strong emphasis on linking education with current clinical issues. The NP/TNP will initiate and participate in research and quality improvement activities and utilise these findings in the provision of clinical service delivery to ensure that practice is based on the most recent and available evidence, with a focus on improved service delivery.

This position is a on a casual basis to assist with leave across the service and operates on a 7 day a week rotating roster with shifts across weekdays, with afternoon and weekend cover required.

This position will be joining The Geriatric Flying Squad Department as the service is currently expanding through Ministry of Health Urgent Care Funding, available until June 2025. This will enable an Allied Health clinician and Nurse Practitioner to provide assessment and management within the GFS service, to the community, 7 days a week and into the after-hours, with the support of the on-call Geriatrician. This will be an important element of the SESLHD Urgent Care Model in providing assessment and management in the older persons home, avoiding unnecessary ambulance use and emergency presentation.

All Category A NSW Health workers are required to have received an annual seasonal Influenza vaccination or have an approved medical contraindication certificate. COVID-19 vaccinations are strongly recommended for all NSW Health workers.

Selection Criteria:

  1. Registered Nurse with AHPRA including endorsement as a Nurse Practitioner as per the Nursing & Midwifery Board Guidelines on endorsement as a Nurse Practitioner
  2. Demonstrated high-level knowledge and advanced clinical nursing skills, including comprehensive assessment, interpretation of diagnostics, implementation of treatment modalities and relevant referral skills applied to a broad range of challenging, complex and changing situations.
  3. Demonstrated ability to perform in a clinical leadership role evidenced by participation in practice development activities including mentoring, education, active participation in committees and policy development
  4. Demonstrated involvement in research and quality improvement that influence and enhance clinical service delivery and outcomes, models of care and evidence-based practice
  5. Demonstrated effective communication and interpersonal skills across a variety of mediums, with an emphasis on service delivery, quality of care and team cohesion
  6. Demonstrated ability to undertake clinical service planning in conjunction with the organisation and optimising professional networking
  7. Demonstrated ability to work with a high degree of autonomy and accountability while also working collaboratively in a multidisciplinary and multi-service environment.
  8. Computer skills with experience using MS Office programs and electronic filing systems
  9. Current C-class driver’s license
