Programs Manager Population Health (HM3) - PFT

• Relevant tertiary and/or post-graduate qualifications or equivalent work experience, or a combination of study and work experience.
• Proven health and clinical service management experience at a senior level including strategic planning, operational management, and financial management skills.
• Demonstrated experience in human resource management including team leadership, staff recruitment, and professional development activities.
• Demonstrated understanding of HARP and other Population Health issues and services and their application to the custodial environment.
• Demonstrated high level organisational, communication (including written and verbal), and information technology (IT) skills, including the development of formal and executive level documentation.
• Demonstrated ability to work autonomously and within a team to motivate and support staff to achieve service and organisational objectives.
• Demonstrated project management skills including all aspects from project initiation to evaluation with the ability to engage multiple stakeholders.
• Proven experience in working to key performance indicators and monitoring outcome measures.
