What you'll be doing
The Transitional Nurse Practitioner will provide high levels of clinically focussed nursing care within the specialty area of palliative care- end of life nursing to improve access of care and address gaps in existing health care services through the provision of a flexible and innovative model of care.
The Transitional Nurse Practitioner will provide clinical and professional leadership as an expert in the clinical field and will undertake comprehensive health assessments and collaborate and consult with other members of the health care team to plan, implement and evaluate care. This integrated approach will support best practice and maximise patient care outcomes.
  1. Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia and…..either • endorsement by the Nursing and Midwifery Board of Australia as a Nurse Practitioner with a minimum of 5 years’ full time equivalent post registration experience with at least 3 years’ full time experience in Palliative Care- End of Life and post graduate qualifications relevant to the area of clinical specialty. • OR TNP: Registered Nurse with at least 5 years’ full time equivalent post registration experience and at least 3 years’ full time equivalent experience in Palliative care –End of Life with post graduate qualifications in Palliative Care or a relevant field or current enrolment in a Masters of Nurse Practitioner course relevant to the speciality.
  2. Demonstrated advanced clinical skills, competence and leadership in the assessment, treatment and management of patients with Palliative Care and End of Life care needs.
  3. Demonstrated understanding of integrated care and proven ability to work collaboratively within a multidisciplinary framework and mixed clinical environment.
  4. Demonstrated knowledge and understanding of current issues impacting on the provision of Palliative Care services in regional and rural areas.
  5. Demonstrated high level advanced communication and interpersonal skills (including the use of coaching) across a variety of mediums and proven ability to write for a variety of documents.
  6. Proven advanced skills in clinical teaching and leadership in strategic and clinical service planning and evidence of the ability to influence organisational and cultural change.
  7. Evidence of high level commitment to quality improvement and evidence based practice.
  8. Proven information technology skills with specific experience in the use of word processing, database and spreadsheet software. Valid unrestricted drivers licence for use in NSW/Australia and willingness to travel in the course of employment
