The Quick Access Response Team (QuART) is a dynamic community based allied health team providing interdisciplinary allied health care and care coordination to patients in their homes to prevent admissions or support earlier discharge from hospital.

The position provides a specialised clinical service to patients/client/consumers across the Illawarra Shoalhaven Local Health District. The position will provide high quality outpatient and/or inpatient services for referred patients/clients/consumers and act as a consultant for health professionals in the area of Allied Health: Nutrition & Dietetics, Occupational Therapy, Physiotherapy and Speech Pathology.
QuART clinicians work within an interdisciplinary model of care to provide 2 weeks of coordinated, at-home, allied health support and intervention so patients can avoid hospital admissions and/or be discharged as early as possible. Clinicians work with an expanded scope across the allied health disciplines in the team to provide a coordinated service for each patient while being the lead for their discipline within the team.
QuART clinicians work closely with each other to support patients as well as Emergency Department clinicians, inpatient teams, outpatient and community services to identify patients appropriate for the service. QuART clinicians also liaise extensively with ISLHD outpatient/community services as well as GPs and external agencies and services to coordinate ongoing care and support for patients to prevent representation to hospital.
QuART is located at both Wollongong and Shoalhaven hospitals with positions specific to each site. Teams may provide support across both sites when required. QuART clinicians work predominantly in the community, visiting patients in their homes and provide a broad range of services, not limited to single disciplines. The team also reach into the hospitals to facilitate discharges when required.
