Where you'll be working
3 Full time roles, each working at one of these sites - Royal North Shore Hospital (RNSH), Ryde Hospital, Hornsby Ku-ring-gai and a Part time role based at Mona Vale Hospital.
What you'll be doing
The Discharge Patient Flow Concierge Lead for each facility will provide high-level speciality support related to care coordination to improve communication, patient and carer experience and to deliver on patient flow outcomes. It will be a centralised team to work with the sites in a coordinated approach. The roles will assist with earlier and comprehensive discharge planning. The position will focus on leading improvement of the discharge patient flow process through collaboration with clinical multidisciplinary teams and provision of expert guidance to ensure efficient hospital operations and high-quality patient care.

Candidates will need to meet the following criteria:
- Relevant tertiary qualifications in a health-related field or relevant equivalent work experience, or a combination of study and experience.
- Demonstrated experience in continuous clinical practice improvement, project management and/or change management.
- Demonstrated management skills to enhance staff performance, develop innovative workplace solutions and empower stakeholders.
- Proven ability to collect and analyse quantitative and qualitative data to evaluate projects and prepare high-level reports.
- Work location preference for full time role only: Hornsby, Ryde or RNSH.
