How to Apply
To be considered for this position, please ensure you address the selection criteria in the application as thoroughly as possible

• Relevant qualifications in a health, disability or social service role.
• Knowledge of Intellectual Disability and/or physical disability with current/recent (within the last 6 months) experience working with people with Intellectual/physical Disabilities.
• Minimum 3 years' experience working in the disability sector, with a strong understanding of navigating the NDIS and NDIA.
• Experience coordinating and communication complex care plan within a health care context, as part of a multi-disciplinary team.
• Current unrestricted NSW drivers licence, ability and willingness to travel across the Far West LHD, frequently staying overnight, sometimes several nights.
