What you'll be doing- Provide expert advice to support staff to deliver patient/client care to people with an Intellectual Disability and to support referred patients in accessing the Specialist Intellectual Disability Health Team based in WNSWLHD if or when required.
- Provide care coordination and navigation for NDIS eligible people who are in hospital, at risk of hospitalisation or where the support will expedite access to NDIS services to relieve pressure on LHD services.
- The position will inform and guide health staff on the delivery of care for people with Intellectual and/or physical Disability and their carers and families in any hospital or site in the Local Health District, including implementation and monitoring of the Disability Inclusion Action Plan (DIAP).

How to Apply
To be considered for this position, please ensure you address the selection criteria in the application as thoroughly as possible
- Relevant qualifications in a health, disability or social service role.
- Knowledge of Intellectual Disability and/or physical disability with current/recent (within the last 6 months) experience working with people with Intellectual/physical Disabilities.
- Minimum 3 years' experience working in the disability sector, with a strong understanding of navigating the NDIS and NDIA.
- Experience coordinating and communication complex care plan within a health care context, as part of a multi-disciplinary team.
- Current unrestricted NSW drivers licence, ability and willingness to travel across the Far West LHD, frequently staying overnight, sometimes several nights.
