What you will bring to the role:
- Current registration with the Australian Health Practitioner Regulation Agency (Ahpra) as a Registered Nurse with a minimum of five years’ experience in an acute setting two years clinical experience in specialty
- Demonstrated effective clinical communication using information technology and sound written, verbal and interpersonal skills.
- Demonstrated ability to work effectively and collaboratively as part of the Multidisciplinary Health Care team
- Proven ability to prioritise and organise own workload
- Demonstrated ability to network and liaise with government and non-government service providers
- Demonstrated ability to apply clinical expertise in patient centred care consultancy with the ability to work independently
- Consistently demonstrates behaviours that reflect the CORE Values; Collaboration, Openness ,Respect and Empowerment of Nepean Blue Mountains Local Health District.
- Understanding and knowledge of current NSW Ministry of Health Policy Directive Admission to Discharge Care Coordination and how this guides practice
