About the role
An exciting opportunity exists to join a service promoting quality of life and dignity for those with a life limiting illness.
The service includes a team of specialist nurses, Social Workers, Occupational Therapists, Dietitian and Palliative Care Doctors. This team is supported by Clinical Nurse Consultants and Nurse Educator in Palliative Care.
This is currently a Monday to Friday service with a telephone support service, after hours and on weekends and works closely with other care providers and services to support clients, their families and carers.
