Are you an independent, proactive and decisive pharmacist with superior clinical pharmacy and communication skills? Then the SESLHD Medication Management Team might be your ideal workplace. 

This role is open to candidates who are looking for Permanent Full-time (up to 38 hours p/week) OR Part-time Hours 

What you'll be doing

The SESLHD Medication Management Team (MMT) provide specialist clinical pharmacy services to the South Eastern Sydney Local Health District (SESLHD) within District Emergency Departments and priority units to optimise safe and appropriate medication usage for patients and staff. MMT Pharmacists promote industry leading practice in the areas of Best Possible Medication History (BPMH), Medication Reconciliation and Medication Review to optimise patient safety and outcomes. These practices includes services delivery and innovation in medication management, including participation in SESLHD Partnered Pharmacist Medication Charting (PPMC) Program and supporting the development of other innovative models of care.

 
The position forms part of a team led by a Team Leader Pharmacist and supported by other pharmacists and assistants/technicians. The position provides medication management activities, working across the SESLHD sites, providing service with rotating rosters across the seven-day week. This may include participating in on-call, afterhours, flexible work hours, weekend and public holiday work. It will be required to rotate through a variety of clinical and dispensary teams. The role will collaborate with all SESLHD Pharmacy departments and their identified site stakeholders to ensure consistent and effective service provision across SESLHD.

South Eastern Sydney Local Health District (SESLHD) is one of Sydney’s largest health districts, covering 468 square kilometers from the CBD and beaches of Bondi to Cronulla, extending to the Royal National Park, and serving around 930,000 residents in areas including Woollahra, Waverley, Randwick, Bayside, Georges River, and Sutherland Shire, as well as Lord Howe Island. SESLHD manages nine hospitals: Prince of Wales, Royal Hospital for Women, Sydney/Sydney Eye, Uniting War Memorial, St George, Calvary Health Care, Sutherland, Garrawarra Centre, and Gower Wilson Memorial, providing comprehensive healthcare across southeastern Sydney.
