Social Worker Acute Services Team - Level 1 / 2 - Prince of Wales Hospital, Randwick

The Role
Prince of Wales Hospital is a 440 bed tertiary referral facility with specialties unique to this campus such as spinal cord injuries and interventional neuroradiology. With the opening of a new Acute Service Building, Prince of Wales Hospital provides state of the art modern facilities aimed at providing an exceptional staff experience and enhanced patient and family outcomes. It includes an expanded Emergency Department and Intensive Care Units, a Community Management Centre, and replacement of multiple existing inpatient wards with contemporary facilities. There will also be new research, education and training spaces, including additional spaces funded by UNSW Sydney, to enable clinical innovation and research, biomedical engineering, and clinical translational research laboratories collocated directly alongside clinical staff providing acute healthcare services.
The hospital is accessible via a light rail service which stops right outside the main entrance, along with a range of bus services. The position sits within the social work acute services team and will provide a social work service to patients and their carers across the acute services team, working in renal, orthopaedics, urology, haemodialysis, endocrinology, community assessment unit, emergency department and intensive care units. The successful applicant will have excellent communication and time management skills with relevant experience, and knowledge being desirable but not necessary with training and support available. As an acute services social worker you will complete psychosocial assessments, provide counselling around adjustment to illness, crisis and bereavement support, crisis intervention, referral to community services and discharge planning.

- Qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award providing eligibility for membership of the Australian Association of Social Workers.
- Demonstrated core competency skills in psychosocial assessments, individual and family casework, crisis intervention, counselling, advocacy and discharge planning.
- A good knowledge of community resources and experience liaising with community organisations.
- Demonstrated high level written and verbal communication, and interpersonal skills, and the ability to proactively engage with patients/clients to enhance service delivery.
- Ability to work as part of a multi-disciplinary team and to exercise independent professional judgement on routine matters, commensurate with years of experience
- Computer skills and the capacity to use health information systems.
- Demonstrated commitment to quality improvement and evidence based practice and a commitment to ongoing clinical and professional learning
- Current drivers licence (or a willingness to travel in accordance with the demands of the position) and the ability to participate in the After Hours Crisis On-Call service, and work across a seven day roster (including weekends).
