What you'll be doing 
As a Social Worker Level 1/2, you will provide comprehensive psychosocial assessment, intervention and advocacy to patients and their families, supporting them through complex health and life circumstances. You'll work independently on routine clinical matters while receiving professional support for more complex presentations and contributing to continuous service improvement.

  • Conduct comprehensive psychosocial assessments and provide counselling, crisis intervention, advocacy and referral to community services.

  • Collaborate with medical, nursing and allied health teams to support patient care and complex clinical decision-making.

  • Manage complex presentations including safeguarding, domestic and family violence, child protection, guardianship and end-of-life care.

  • Participate in clinical supervision, professional development, quality improvement activities and student supervision.

  • Maintain accurate clinical documentation and contribute to evidence-based, patient-focused care.

Essential Criteria

  • Qualification: Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW). 

 

Selection Criteria

  1. Demonstrated understanding of psychosocial issues associated with acute and chronic illness.

  2. Demonstrated capacity to provide counselling and case management with patients/families with illness adjustment, end of life care and with complex discharge issues

  3. Demonstrated clinical experience or understanding of the psychosocial impact of grief, loss and bereavement.

  4. Demonstrated understanding of issues associated with working with Culturally and Linguistically Diverse client group.

  5. Demonstrated well-developed time management skills with excellent written and verbal communication, including report writing skills and high-level computer literacy.

  6. Demonstrated ability to work with minimal supervision and as part of a multidisciplinary team including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels

  7. Knowledge of quality assurance and / or clinical practice improvement activities

  8. Unrestricted drivers licence and availability to participate in the After-Hours Crisis On Call Social Work Service.
