About the Opportunity
An exciting opportunity exists for an experienced and dynamic leader to join SWSLHD as the Senior Manager, Connected Care (SMCC). This pivotal role is responsible for leading district-wide projects, driving strategic initiatives, and championing digital innovation to enhance healthcare delivery for our patients.You will report to the Associate Director of Clinical Operations within the District Operations and Performance portfolio. As a key member of the team, the SMCC will provide expert leadership in financial and performance reporting, virtual care funding models, and technology-enabled healthcare solutions. What You'll be Doing
As part of the SWSLHD Clinical Operations team within the Clinical Operations and Performance portfolio, the SMCC collaborates with senior leaders across the organisation.
Key responsibilities include:
- Strategic Leadership – Oversee financial and performance reporting, information management, and clinical systems to support operational priorities. Lead monitoring, evaluation, and reporting efforts, informing future funding models and projects.
- Service Performance & Reporting – Develop and coordinate strategies to enhance service performance monitoring and reporting for Connected Care’s strategic goals. Support the ADCO in funding classifications, costing, and financial reporting.
- Operational & Team Management – Oversee the Connected Care team, ensuring effective operations and alignment with organisational objectives.
- Technology & Innovation – Lead the selection and implementation of technology that supports virtual models of care and in-person clinical practice. Work closely with clinicians and Health Informatics leads to ensure technology enhances patient outcomes.
This role is pivotal in driving Connected Care’s success by integrating performance insights, strategic leadership, and technological advancements to improve healthcare delivery.

- Relevant tertiary qualifications or relevant equivalent work experience in health management, project management or a related discipline, and/or appropriate experience within the healthcare system.

- In this role, you’ll be responsible for allocation of funding models and performance reporting for initiatives. How have you previously approached financial oversight and data-driven decision-making in a healthcare setting?
- Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
