About the role:

New England NorthWest Mental Health Service (NENW MHS) seek a dedicated and passionate Rehabilitation Coordinator to join our team!

This role will provide leadership and expert consultation within the service in regard to clinical rehabilitation. In your role as a Rehabilitation Coordinator, you will:

  • Deliver high quality evidence based clinical care to consumers in the NorthWest
  • Provide and coordinate clinical rehabilitation services that promote recovery and reduce the disability and functional decline associated with mental illness utilising a range of targeted evidence-based interventions
  • Develop and maintain a network of intersectoral links and partnerships within the sector
  • Be available to guide the day-to-day practice and coordination of clinicians
  • Be a point of contact for escalation with clinical and operational matters
  • Ensure that patient care is delivered in accordance with professional, legal, and ethical standards, while championing evidence-based practices
  • Manage service delivery, monitor clinician workloads and performance, and provide ongoing supervision and support to maintain seamless care
  • Contribute to operational planning, meet service targets and KPIs, and ensure exceptional service delivery

 
To aid your success in this role, you will have access to support and education from key district support positions. Clinical supervision is available and encouraged.

Where you’ll be working:

Our Mental Health Clinicians are based at our Tamworth, Gunnedah, Narrabri, and Moree sites, providing service across the NorthWest catchment using a combination of face to face and telehealth services. This position will be site negotiable within the NorthWest.

You will have opportunity to work with the NorthWest Community Manager, NorthWest Clinical Coordinator and the NENW MHS leadership team to maximise the effectiveness and enhance the care provided to communities.

About You:

This role will see you become an integral part of our skilled multidisciplinary team – you will practice autonomously, as well as in collaboration with others to access and manage patients within the clinical context, using expert knowledge and skill at an advanced level.

We are looking for someone who is organised and flexible, with the ability to adapt to changing needs and priorities. You will have a passion for recovery orientated care and have experience building strong relationships with multiple agency partners to enhance collaboration and deliver integrated services across sectors.

This role will require you to be willing to travel as required to visit our clinicians in the smaller remote, rural areas. You will bring proven leadership experience, ensuring effective operations and a positive work culture across all shifts.

Requirements:

  • Must hold a qualification in Nursing, Psychology, Social Work, Counsellor (Degree in Bachelor of Health Science Mental health, Djirruwang Program), or Occupational Therapy which provides eligibility for membership of the relevant professional association and/or provisional or general registration through the Australian Health Practitioner Regulation Agency (AHPRA) as relevant.

  • To be eligible for permanent employment in this position you must be an Australian Citizen, or a permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport.
