For this role, we are looking for a health care professional with:
- Relevant tertiary and/or postgraduate qualifications in mental health
- Current registration with the Australian Health Practitioner Regulation Agency, or membership to professional associations where registration is not offered
- Advanced interpersonal skills and leadership experience
- Experience with multi-agency liaison and inter-service collaboration.

The Role
The Manager Community Transitions Team (CTT) is primarily responsible for the day-to-day operations of the Community Transitions Team. The CTT is one of three services falling under the remit of NSW Community Forensic Mental Health Services. The CTT is a multidisciplinary assertive transition support service that is based in the community but provides prison in-reach services to people who experience complex and severe mental illness who are transitioning from custody to the community. The CTT develops transition plans to facilitate safe handover of clinical information, establish necessary therapeutic supports for patients, and provide follow-up community care to especially vulnerable people leaving custody.
The Manager CTT will provide operational oversight, resource management and support to the multidisciplinary team while maintaining a capped clinical caseload. The caseload will include those people who are assessed as higher risk and will be an essential component of managing the service’s leave relief. This role will be required to support the CTT in the delivery of specialist transition services that utilise risk assessment and management principles to guide their transition planning and recommendations. The role will also require skills in strategic planning, data collection, budgeting, program evaluation and people management expertise at a senior level.
The Manager CTT will work in partnership with the Local Health Districts, primary health networks, Mental Health Review Tribunal, Department of Community and Justice and other government and non-government organisations (NGOs) across NSW to ensure evidence based, forensic mental health care is delivered.

The Justice Health Olympic Park (JHOP) office is located at 10 Herb Elliot Avenue, Sydney Olympic Park. Designed for inter-departmental communication and collaboration, the office provides large open spaces and an abundance of natural light. An array of restaurants and cafés are a short walk away.
Public transport is readily available including buses, trains, and ferries. Easy road access is provided via the M4 and Parramatta Road via Homebush Bay Drive. 35 km of bicycle paths surround the site and both bicycle storage and shower amenities are available inside.
