About the role
Provide support, advocacy and education to patients with Type 2 Diabetes enrolled in our program, while working within a dynamic team to provide virtual care to patients with Type 2 diabetes in the District wide program.
Support patients and their families/carers broaden their understanding and management of their chronic disease.
Work in an LHD position in a flexible work environment to develop partnerships across the health system and with community, while supporting the local Health workforce to enhance their knowledge of Type 2 diabetes. Participate in innovative models of care.
