About the role:
Based at Kurri Kurri, the Community Dementia Nurse plays a pivotal role for patients and their families in getting the appropriate level of support during mental decline. On any given day, you will be triaging referrals for patients within the local area; providing telephone consults and/ or home visits; linking in with other dementia support services; and providing education to families and carers. If you are an emotionally intelligent Registered Nurse who can appreciate the complexities that come with people living at home with dementia, then we want to hear from you.
You won’t be working alone. You will be supported by the Nurse Manager of Aged Care Services, as well as the Clinical Nurse Consultant (CNC) as you step into the role. There is ease of access to the dementia advisory service and Nurse Practitioner in Aged Care, as well as the broader multi-disciplinary team that sits within the aged care portfolio.

Requirements:
1️⃣ Current Authority to Practice as a Registered Nurse with AHPRA
2️⃣ Eligibility to drive in NSW and the ability and willingness to travel for work purposes.
