What you'll be doing
This position works with individuals and their families, health practitioners and communities to support patients and link them to health and other services. This position requires good skills in communication, identifying and solving barriers in access to care, collaborative problem solving and knowledge of referral to health services.
The primary purpose of MNC Virtual Care is to reduce preventable admissions and decrease length of hospital stay through Urgent Care service delivery, early intervention, and care navigation to people who have been identified as at risk of presentation and hospitalisation for low acuity conditions that can be safely managed over a virtual platform.

Salary will be accordance with NSW Health State Awards for the advertised classification/s. These awards are available at http://www.health.nsw.gov.au/careers/conditions/pages/default.aspx.
A TALENT POOL MAY BE CREATED FOR FUTURE ROLES AND WILL REMAIN VALID FOR 18 MONTHS.
- Successful completion of a relevant Certificate III (or higher) qualification in Health Services Assistance, for example the current course HLT33115 Certificate III Health Services Assistance (Assisting in Nursing Work in Acute Care), or having equivalent experience or have completed 1 or more years training as an undergraduate nursing student. OR Appropriate skills and abilities to be classified as an Allied Health Assistant (AHA). To meet the requirements for Level 2 AHA: demonstrated experience working as an Allied Health Assistant for a period of 12 months or more and/or Certificate III in Allied Health Assistance (or equivalent qualification) or more years training as an undergraduate Allied Health student. If the applicant does not meet requirements above, they will commence service at Level 1 AHA.
- Demonstrated ability to effectively communicate and engage with patients/consumers, carers and the multidisciplinary health care team to identify and ensure responsiveness of care needs.
- Demonstrated effective clinical communication and willingness to learn new information technology applications, including written, verbal communication and interpersonal skills including the ability to communicate with a diverse range of people, utilizing a broad range of technology platforms.
- Demonstrated evidence of the use of critical thinking as it relates to the delivery of evidenced based care
- Demonstrated ability to work under supervision and provide care within a multidisciplinary care setting.
- A thorough knowledge of scope of practice and a demonstrated ability to work within scope boundaries.
- Current unrestricted NSW drivers license and willingness to travel during course of working day.
