About the role
An exciting opportunity to join our Western Virtual Patient Flow Access and Command Centre.
Delivering one service across many places, the Clinical Nurse Specialist will partner with our existing patient flow, inpatient and Connected Care in Community teams, supporting discharge coordination processes to meet key performance outcome measures.
This Clinical Nurse Specialist position will be a rotating roster, morning and afternoon shift, 7 days a week, supporting a coordinated multi-disciplinary team approach.
