About Your Role
The Virtual Palliative Care Patient Liaison Officer CNS2 Sector 1 works virtually as a Specialist palliative care patient liaison within the Specialist Palliative care team across MLHD hospitals and MPS within Sector 1;
- Sector 1 Includes Griffith, Tumut, Batlow, Tumbarumba, Deniliquin Cluster, Corowa Cluster and Leeton Cluster.
Mercy will continue to provide palliative care services to Young, Harden and Boorowa. This is not a Discharge planner role.
They are fundamental in coordinating the delivery of palliative care within the inpatient setting and ensuring the smooth transition between the community and acute environment. Their role encompasses; introduction to palliative care for clients referred to the service, symptom management advice, end of life care coordination (including discharge into the community) and transition to home/residential aged care.
The Palliative Care Liaison is a leader in the palliative care space and works collaboratively as part of the MLHD Specialist Palliative Care Team
They have excellent communication skills and liaise frequently with clients, families, and health professionals across the interdisciplinary team. As part of their role, they provide a high level of education in a formal and non-formal capacity. They continuously engage in ongoing professional development and research opportunities
