The Sutherland Hospital is now recruiting to Aged Care Service Emergency Team (ASET).

Your new role:
The Clinical Nurse Specialist is a Registered Nurse who has responsibility for the coordination and clinical support of emergency department based aged care services and rapid response aged care community services within the division of Southcare (Aged and Extended Care).  Duties include efficient and effective management of older people who present to the Emergency Department and reside in the community. 

Where you'll be working:
ASET provides a multidisciplinary consult service for patients 70 years and older who present to the Sutherland Hospital Emergency Department. ASET work collaboratively with Emergency Department staff to achieve optimal care and management of the elderly patient.

ASET assess and identify complex issues, advocate for patients and their carers’, refer and facilitate access to other relevant services if medically cleared from the Emergency Department.

      Selection Criteria: 

      1. Current registration with the Nursing & Midwifery Board of Australia (AHPRA)
      2. Relevant post-registration qualification and at least 3 years' experience working in the working in the Aged Care clinical area, or such other qualifications or equivalent experience deemed appropriate
      3. Demonstrated ability to manage complex clinical care and service delivery issues and problem solve through the development and use of clinical management plans
      4. Demonstrated ability to exercise extended autonomy of decision making while working as part of a multidisciplinary team and apply professional and ethical boundaries when dealing with complex situations
      5. Demonstrated skills in the development & facilitation of patient and carer education [and staff as required]
      6. Demonstrated high level written and verbal communication, interpersonal skills and the ability to identify key messages, issues and concerns when communicating with others
      7. Detailed knowledge and understanding of harm minimisation and evidence of leadership in safety and quality innovation
      8. Demonstrated competence in the use of health-related information technology.
