Salary: $74,317.94 - $104,336.27 (pro-rata) + super.

About the role
We are excited to offer an opportunity to join our dynamic and growing team supporting clients through the Commonwealth Home Support Programme (CHSP).

This role provides a rewarding chance to make a meaningful difference by delivering person-centred care to older people living in the community.

  • Play a key role in providing high-quality nursing care to clients referred to our entry-level aged care services.
  • Work within a supportive and collaborative multidisciplinary team, you will undertake comprehensive clinical assessments, develop care and discharge plans, and deliver therapeutic interventions tailored to each client's individual needs.
  • Case management for older clients in their homes, supporting them to maintain independence, safety, and quality of life within their community.
  • We operate Monday to Sunday, including public holidays.

About you

  • Current registration as a Registered Nurse (Division 1) with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Minimum of 2 years' experience in community setting.
  • Current unrestricted Class C Driver's Licence (or above).
  • Demonstrated ability to work collaboratively within an interdisciplinary team, with strong communication and interpersonal skills
  • Commitment to ongoing professional development and demonstrated computer literacy, including email and electronic patient record systems.
