What you'll be doing
The Community Diabetes Nurse Consultant (CDNC) leads diabetes management initiatives in Western Sydney, supporting GPs and allied health professionals to enhance diabetes care in outpatient settings. This role combines clinical leadership, education, research, and strategic planning to improve patient outcomes.
- Lead and implement a diabetes education program across multiple sites.
- Develop and refine complex diabetes services in the local health district.
- Build capacity of GPs and allied health providers in managing type 2 diabetes.
- Initiate and disseminate locally based diabetes research.
- Provide expert clinical advice and coordinate diabetes case conferences.
How to Apply
Essential Criteria:
- Registration: Current Registered Nurse Division 1 with the Australian Health Regulation Practitioner Regulation Agency (AHPRA) with 5 years post graduate experience including 3 years in diabetes.
Selection Criteria:
- Previous experience working in primary care and the ability to use software applications.
- Demonstrated ability to implement changes and develop direction in clinical practice by the initiation, utilisation and review of evidence based research.
- Experience with contributing to strategic and clinical service planning and the management of organisational and cultural change.
- Demonstrated high level advanced communication and interpersonal skills across a variety of mediums and proven ability to write for a variety of documents.
- Proven advanced skills in clinical teaching, competency in the use of contemporary information systems and evidence of the ability to develop, implement and evaluate specialty education programs.
- Demonstrated ability to work collaboratively in a multidisciplinary framework within I&CH and the LHD, and addition other service providers.
- Current Drivers Licence valid within Australia and willingness to use for work purposes.
