What you'll be doing

As a Registered Nurse in this role, you will provide high-quality, person-centred nursing care to patients with chronic and complex conditions, ensuring safe, effective and coordinated care across the patient journey within the Chronic and Complex Care Ward.

 

In this role you will be:

 

  • Delivering individualised, evidence-based nursing care to maximise patient outcomes.

  • Responding to changing patient conditions and managing competing clinical priorities.

  • Coordinating discharge planning and making appropriate referrals to support continuity of care.

  • Supervising, mentoring and supporting nursing staff and students within your scope of practice.

  • Advocating for patients and families while managing resources efficiently and ethically.

Essential Criteria:

  • Registration: Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) (NSW Registered Nurse – Division 1) 

 

Selection Criteria:
 

  1. Demonstrated recent nursing experience relevant to the position and commitment to quality improvement, practice development and evidence-based practice

  2. Demonstrated application of evidence based practice and critical thinking to patient care

  3. Demonstrated ability to lead, supervise, delegate, and negotiate with others to deliver quality nursing care

  4. Demonstrated effective clinical communication through the use of information technology, written and verbal means,; and sound interpersonal skills including the ability to communicate with a diverse range of people.
