Are you a skilled and motivated Registered Nurse with a strong background in acute care and a passion for diabetes management? Join the collaborative team at the Diabetes Centre at South Western Sydney Local Health District (SWSLHD), delivering innovative, person-centred care to individuals living with or at risk of diabetes. This position offers a unique opportunity to work across both outpatient clinics and referral pathways from inpatient wards and the Emergency Department, supporting complex diabetes care across the continuum. You will be part of a highly experienced multidisciplinary team, including Endocrinologists, Nurse Practitioners, Clinical Nurse Consultants, CNSs, Credentialled Diabetes Educators, Dietitians, Psychologists, and Podiatrists.As a key member of the team, you’ll contribute to direct clinical care, patient education, and service delivery, while also participating in quality improvement initiatives, research, and project work to enhance service outcomes.

Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.
1. Current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA), with a minimum of 5 years’ full-time equivalent experience in an acute hospital setting.
2. Credentialled Diabetes Educator (CDE) certification or active progression toward becoming credentialled, with demonstrated commitment to professional development in diabetes care.
3. Proven ability to work collaboratively within a multidisciplinary team, including Endocrinologists, Diabetes Nurse Specialists, Dietitians, Psychologists, and Podiatrists, in the delivery of coordinated, person-centred care.
4. Demonstrated experience or involvement in quality improvement, research, or service development, with a focus on enhancing outcomes in chronic disease management or integrated models of care.
