About the Opportunity

Make a real difference in someone’s recovery and help older people get back to doing what matters most.

Join the Transitional Aged Care Program (TACP) as a Registered Nurse, supporting older people as they transition from hospital back into their homes and communities. You’ll play a key role in helping patients rebuild their strength, mobility and confidence, regain independence and access the ongoing care they need.

Based within the Lower Mid North Coast, this rewarding community-based role offers the flexibility to work from Taree, Forster or Gloucester Community Health, with opportunities to support patients across surrounding communities. You’ll work closely with patients, families, carers and a multidisciplinary team to deliver personalised care that makes a lasting difference.

You’ll also benefit from a supportive leadership team, dedicated administrative assistance and training to help you grow your skills and confidence in transitional care.

What You Will Be Doing

As a Registered Nurse, you will coordinate and deliver person-centred care that helps older people recover following hospitalisation and remain safely at home.

Your responsibilities will include:

  • Completing patient assessments and coordinating referrals from hospital.

  • Developing care plans that support recovery, mobility and independence.

  • Managing patient care and monitoring progress throughout the program.

  • Working with patients, families and carers to establish meaningful goals.

  • Coordinating ongoing support and safe transitions from the program.

  • Collaborating with nursing, allied health and community support teams.

  • Managing a varied community caseload across the Lower Mid North Coast.

About You

You’re a compassionate and proactive Registered Nurse who enjoys building relationships and empowering older people to achieve their goals. You’re confident working autonomously in the community, making sound clinical decisions and coordinating care with a multidisciplinary team.

You will bring:

  • Current registration as a Registered Nurse with AHPRA.

  • Strong clinical assessment and care coordination skills.

  • Effective communication, organisation and time management.

  • The ability to work independently, adapt to changing needs and prioritise competing demands.

  • A genuine passion for supporting older people to regain their independence.

  • Experience in community nursing, rehabilitation, aged care, discharge planning or transitional care will be highly regarded.

Ready to make a difference? Join a team that helps older people recover with confidence, maintain their independence and enjoy the best possible quality of life at home.
