The Primary Care Outreach Geriatrician will provide advice, education and support to primary care providers (GPs, Practice Nurses) servicing community-dwelling older people in the Lower North Shore region, in addition to inpatient and outpatient duties as a staff specialist geriatrician at Royal North Shore Hospital.
This position will support the Integrated Care, Collaborative Commissioning and Virtual Hospital initiatives by providing specialist outreach to primary care to enable patient care planning and education opportunities between GPs and the geriatrician.
The role will include collection of appropriate data on the service provided to GPs and outcomes for patients.
Purpose of the Position
The position will provide clinical consultation services within NSLHD and will contribute to the teaching and training of medical students, graduates and other health care professionals and to research, evaluation and quality improvement activities within NSLHD.
Selection Criteria
- MBBS or equivalent, currently registered or eligible for registration with the Medical Board of Australia. Applicants must have Fellowship of the Royal Australasian College of Physicians or recognition as a specialist by the Medical Board of Australia.
- Consistently demonstrates behaviours that reinforce the CORE Values of our organisation; Collaboration, Openness, Respect and Empowerment. Demonstrates these behaviours with all stakeholders; colleagues, direct reports, as well as our patients and consumers, and those that care for them.
- Appropriate experience to practice as a Senior Medical Practitioner in Geriatric Medicine.
- Demonstrated experience in community-based Geriatric Medicine, preferably as part of a multidisciplinary community-based team.
- Demonstrated excellent interpersonal and communication skills.
- Current NSW Driver's licence.
- Demonstrated computer literacy skills and knowledge of Microsoft Office and email applications.
