Remuneration:  

·       Clinical Nurse Specialist Grade 2: $58.82 - $60.75

·       Counsellor Level 4, Social Worker Level 4: $63.81 - $65.35 per hour

What you'll be doing 
As a KEYS Navigator, you’ll coordinate services for vulnerable families, working across multiple agencies to deliver integrated, person-centred support. You will play a vital role in building partnerships, navigating service gaps, and ensuring families receive coordinated care.

·       Act as the central point of contact for families, screening and coordinating service provision

·       Collaborate with KEYS navigators and agencies to understand family needs and service gaps

·       Support referrers and advocates by providing information and guidance on family care plans

·       Develop and maintain strategic partnerships to ensure timely, culturally appropriate services

·       Monitor family outcomes and adapt service delivery to respond to changing needs

Essential Criteria:

·       Registration: Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) for Nurses or Tertiary qualification in Social Work or Counselling 

 

Selection Criteria: 

1.     Relevant tertiary qualifications in counselling or social work and eligibility for membership of the relevant professional association or current Registered Nurse Division 1 National Registration (AHPRA) relevant post-registration qualifications in child and family health or paediatrics or adolescent / youth health and at least 3 years’ experience working in with children, young people and families, or such other qualifications or equivalent experience deemed appropriate by WSLHD.

2.     Demonstrated experience in the delivery of services to vulnerable and at risk families with children aged up to 5 years

3.     Demonstrated experience and / or understanding of working with parents experiencing severe and complex parental psychosocial and mental health problems with demonstrated knowledge of the impact of psychosocial and mental health problems on parenting, parent –infant relationships and child development.

4.     Demonstrated understanding of the impact of violence, abuse and neglect on child development and the supports that are required by families and their children to address these impacts

5.     Ability to Identify and triage the health needs of children and families and assist in coordination of appropriate early intervention, assessments, and referral to services and programs.

6.     Demonstrated high level ability to build effective relationships and partnerships with internal and external stakeholders including the ability to communicate, consult and negotiate effectively

7.     Demonstrated effective planning, organisational and problem solving skills and ability to work in a changing environment, to prioritise workload and meet deadlines

8.     Excellent interpersonal skills including demonstrated conflict resolution and conflict management skills.
