What you'll be doing
In this dynamic and rewarding role, you will deliver high-quality social work services to patients and clients across the continuum of care while supporting critical workforce demand across WSLHD services.
Provide evidence-based social work assessment, intervention and care planning
Work independently within your level of experience and seek support when required
Collaborate with clinicians, carers and multidisciplinary teams to optimise outcomes
Support patients and families experiencing social, emotional and psychological distress
Maintain accurate clinical documentation, activity reporting and professional supervision requirements

Essential Criteria:
Qualification: A bachelor’s degree in Social Work
Registration: eligibility for membership of the Australian Association of Social Workers.
Selection Criteria:
Demonstrated understanding of the psychosocial issues associated with acute and chronic illness.
Demonstrated capacity to provide counselling and case management with patients/families with illness adjustment, end of life care and with complex case management and discharge issues.
Demonstrated ability to work with minimal supervision and as part of a multidisciplinary team including the ability to interact constructively and collaboratively with a diverse range of stakeholders throughout all organisational levels.
Demonstrated well developed time management skills with excellent written and verbal communication skills including report writing skills and high level computer literacy.
Demonstrated ability to be agile and adaptable and provide high quality clinical services across a broad range of clinical areas at short notice.
Demonstrated understanding of issues associated with working with a culturally and linguistically diverse client group.
Demonstrated commitment to professional development, evidenced based practice, and continuous improvement.
Current unrestricted NSW Driver Licence (or equivalent).
