Employment Type: Temporary Full Time, 38 hours per week until 30 June 2027(willing to accept a 6-month contract) 

Remuneration: 
Clinical Nurse Specialist Grade 2: $2,387.29 - $2,465.80 per week
Counsellor Level 4: $2,424.82 -$2483.60 per week
Occupational Therapist Level 4: $2,424.82 -$2483.60 per week
Senior Psychologist:$2,375.94 - $2,574.58 per week
Social Worker Level 4: $2,424.82 -$2483.60 per week 
In addition to base salary, all roles attract 12% Superannuation + Salary Packaging

This is a great opportunity to lead the Aboriginal Mental Health Transfer of Care Team across SWSLHD, supporting Aboriginal consumers as they transition safely from hospital to community care.

You will guide a team to coordinate culturally responsive, wraparound supports and strengthen partnerships across mental health services, ACCHS, primary care, and community organisations. The role also plays a key part in improving system integration, reducing service gaps, and embedding culturally informed, strengths-based care across the district.

The position is not identified, and we warmly welcome applications from staff who have experience working with Aboriginal consumers in a mental health setting and are passionate about delivering culturally safe and responsive care.

You are an experienced clinician and strong team leader who is passionate about improving outcomes for Aboriginal people. 
You bring: 

  • Experience working with Aboriginal consumers in mental health settings 
  • Strong leadership and team coordination skills 
  • A commitment to culturally safe, trauma-informed care 
  • Ability to build effective partnerships across services and sectors 
  • Experience in care coordination, discharge planning, or service navigation
  • Strong organisational skills and a systems-focused approach

  • How have you previously led initiatives or teams to ensure culturally responsive care for Aboriginal mental health consumers, particularly during transitions from hospital to community settings?
  • Describe a time when you collaborated with multidisciplinary teams and external stakeholders to develop or improve systems that support effective transfer of care. What strategies did you use to ensure cultural considerations were embedded in the process?
  • Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment.
