Join the dedicated Social Work Aged Care Team at Nepean Hospital, where you will play a vital role in supporting patients and their families within the aged care wards. This position offers the opportunity to provide a high-quality social work service that addresses the complex interplay between individuals, their external environments, and the hospital treatment process. Social Work at Nepean Hospital understands that illness and hospitalisation involve significant personal and relational adjustments, often accompanied by a loss of control and independence.
As part of a multidisciplinary health team, you will deliver services including crisis intervention, psychosocial assessments, general counselling, advocacy, case management, therapeutic and grief/loss counselling, and complex discharge planning. Your work will support both patients and the organisation by identifying and resolving psychosocial challenges that impact recovery and transition through the hospital system. By facilitating smooth transitions and addressing emotional and social needs, you will contribute to the overarching goal of patient well-being.

What you will bring to the role
- Degree in Social Work with eligibility for membership of the AASW.
- Demonstrated understanding of the psycho- social impact of unexpected hospitalisation due to illness, accident or trauma
- Demonstrated ability to complete psychosocial assessments and to formulate and implement appropriate discharge and care plan in a short time frame for patients in an inpatient setting.
- Experience in working as a social worker in an acute aged care setting including experience with NCAT, Guardianship Division applications and hearings, elder abuse and complex family situations.
- Demonstrated understanding of the effect and impact of decreasing independence on carers and family.
- Demonstrated ability to work as part of a multi-disciplinary team.
- High level of written and oral communication skills.
