Senior Clinical Program Coordinator - 
Pathways to Community Living Initiative


The Pathways to Community Living Initiative (PCLI) is a coordinated statewide approach to supporting people with significant and persistent mental illness (SPMI) and complex needs who have had long hospital stays, are at risk of long stays or experience frequent admissions; to re-establish or maintain their lives in the community.

The PCLI Senior Clinical Program Coordinator will work in partnership with multidisciplinary mental health teams and develop partnerships with Community Managed Organisations, primary health care and other government and non-government organisations to provide a collaborative approach to development of models, pathways and access to clinical care through PCLI. The Clinical Program Coordinator will promote integration and coordination of day-to-day work practices and will provide seamless advice, assessment and support to multidisciplinary teams.

Position Classification: Clinical Nurse Consultant - Grade 1, Occupational Therapist - Level 4, Psychologist, Senior or Clinical Psychologist, Social Worker - Level 4
Remuneration: $77,358.00 - $133,156.00 per annum (+ super +17.5% leave loading where applicable)

Our ideal candidate will demonstrate:

  1. A relevant tertiary qualification (Nursing, Psychology, Social Work or Occupational Therapy) and current registration with the Australian Health Practitioner Regulation Agency (AHPRA – Nurses, Occupational Therapists, Psychologists) OR eligibility for membership of the relevant disciplines peak body (AASW – Social Workers).

  2. A current Class C Drivers Licence – with willingness to travel across the district for work purposes.

  3. High level of knowledge and understanding of complex Mental Health conditions and the impacts this has on people returning to community living

  4. Desirable but not essential: Knowledge of the National Disability Insurance Scheme (NDIS) and the ability to provide support with matters around NDIS.
