Where You’ll Be Working
The Transition to Residential Aged Care (TRAC) Unit is a 20-bed ward where acute care nursing meets residential aged care. Our patients have completed their acute medical treatment and are transitioning to permanent aged care when it is no longer safe for them to return home.
The team works closely with patients, families and carers to support this significant life transition. The unit culture is warm, supportive and highly collaborative, with a strong focus on dignity, compassion and person-centred care during a pivotal time in patients’ lives.
About the Role
As a TRAC nurse, you’ll provide high-quality geriatric nursing care, including palliative care and social and emotional support, to patients as they transition into residential aged care. You’ll play a key role in supporting patients and families through what can be a challenging and emotional period, ensuring care is delivered with empathy, respect and professionalism.
You’ll be well supported in this role, with dedicated education support available five days per week to help you build confidence, develop your skills and grow into the specialty. This is a rewarding opportunity for nurses who value meaningful connection, holistic care and making a difference beyond clinical tasks alone.
About You
You have Current Authority to Practice as a Registered Nurse with AHPRA. You bring a genuine passion for aged care nursing and a compassionate, patient-centred approach to your practice. You value teamwork, communicate with empathy, and are committed to supporting patients and families through complex transitions.
