About the Opportunity
An exciting opportunity is available for a compassionate and motivated Registered Nurse to join the Healthy at Home service. This unique community-based program delivers the right care, at the right time, in patients’ homes—supporting them to remain safely in the community and avoid unnecessary hospital admissions.
The Healthy at Home service offers a dynamic and rewarding work environment focused on delivering high-quality, patient-centred care in the community. You will work closely with a multidisciplinary team to support vulnerable patients with complex needs, ensuring continuity of care outside the hospital setting.
About the Role
In this role, you will provide clinical nursing care to patients in their own homes as part of the Healthy at Home program. Your responsibilities will include conducting comprehensive nursing assessments aimed at preventing hospitalisation, performing clinical observations and geriatric assessments, wound care, pathology collection, and liaising with Geriatricians, Medical Officers, GPs, and other services involved in the patient’s care.
You will be a valued member of the multidisciplinary team, contributing to safe, coordinated, and holistic care for patients in the community.
About You
You are an accredited AHPRA Registered Nurse who is compassionate, open-minded, and supportive, with a genuine passion for caring for vulnerable patients in the community. You have strong communication skills and the ability to engage effectively with patients, families, and members of the multidisciplinary team.
A comprehensive orientation to the Healthy at Home service will be provided on commencement to support your transition into the role.
