Where you'll be working
The Residential Aged Care In-Reach Service sits within the Hospital in the Home (HITH) team, providing integrated, person-centred care to prevent hospital presentation for vulnerable residents of aged care facilities across the Maitland area and Greater Newcastle. You will be based in Newcastle to provide physiotherapy services to the team via telehealth and face-to-face visits. Newcastle and the Lower Hunter are renowned for the relaxed lifestyle, vibrant café culture, thriving arts scene and natural beauty. The Opportunity
As a Physiotherapist in the team, you will work closely with health professionals based in aged care facilities, acute hospitals and other community health services to provide safe, compassionate care to clients, families and carers. You may be required at times to work with the wider multidisciplinary team across Integrated Care Services. Your responsibilities will include:

This position is line managed by the Allied Health Service Manager for HITH and professional support is provided by the Community Physiotherapy Manager in Integrated Care, Hunter & Lower Mid North Coast. Clinical support is provided by other members of the team. Orientation will be provided to ensure a smooth transition into the role.

 Requirements 

1. Current Authority to Practice as a Physiotherapist with AHPRA 
2. Eligibility to drive in NSW and the ability and willingness to travel for work purposes which may involve driving long distances.
